Adult Day Trainer/Driver

Pay: $17.50 per hour

Job description:

The Entry Trainer for the Upper Keys assists clients in developing basic life skills and appropriate behaviors in both the community and their homes. This role organizes and carries out scheduled activities while maintaining communication with established clients and their families to support ongoing training and services. The Entry Trainer coordinates inclusion activities with clients, families, and necessary staff, provides instruction in basic life skills and appropriate community behaviors, and, when appropriate, provides training for families. Responsibilities include identifying materials or programs needed for each client, implementing instructional programs, monitoring client behavior, and completing required program documentation. Reports client and family needs to the Program Coordinator; attends staff meetings and agency-sponsored activities as needed; coordinates and oversees volunteers assisting during inclusion activities; and transports clients to and from community inclusion events.

*Minimum Requirements: A valid Florida Class D driver’s license. Must have a high school diploma or GED.

Preferred Experience: A two-year degree or one year of experience working with individuals with developmental disabilities or in a related field.


Client Medical Services Assistant, Certified Nursing Assistant (CNA)

Pay Range: Competitive, depending on education and experience.

Job description:

The Client Medical Services Assistant (CMSA/CNA) supports individual client services in areas including dental, medical, speech therapy, physical therapy, psychiatric and psychological services, and visual and hearing care. The position also serves as a liaison between clients, families, and professional support providers to ensure service coordination. The CMSA/CNA supports the CMS in the management of medical services for clients residing in agency group homes and supported living environments. Includes assisting with scheduling appointments, accompanying clients to professional services when needed, and reporting treatment recommendations to the CMS Manager.

Other responsibilities:
-Maintaining ongoing written records of professional services, entering notes into iConnect, updating individual client information sheets, and maintaining medical information in appropriate files.
-Support medication administration when the Medical Services Coordinator is unavailable during scheduled medication passes.
-Position shares 24-hour on-call coverage with the Medical Services Coordinator to assist staff with health-related questions regarding MARC clients outside of regular business hours and performs other duties as assigned.

*Minimum Requirements: Must have a high school diploma or GED. Active CNA license with the state of Florida.

Preferred Experience: Proficient in computer skills or willing to become proficient with outside programs such as iConnect and Therap.

 

Supported Living Coach

Pay: $21.00 per hour

Job description:

The Supported Living Coach works with the coordinator to develop and implement a graduated program that assists individuals in transitioning from existing group or family homes to an independent living situation. The coach provides training and support in all aspects of community living for individuals who have moved into an independent apartment or privately owned home. This role focuses on creating an environment that recognizes and respects each client’s individual needs and their ability to make informed choices. The Supported Living Coach provides the assistance and training individuals need to live successfully in the community, which may include arranging in-home supports, assisting with furnishing and moving into a new apartment, connecting clients with community resources, supporting the development of daily living skills, and providing or coordinating transportation, leisure opportunities, financial assistance, or training in these areas.

*Minimum Requirements: Four (4) year college degree or equivalent experience in social services.

Community Inclusion Trainer

Pay: $17.50 per hour

Job description:

The Community Inclusion Trainer is responsible for training clients in social skills and supporting their participation in community inclusion activities. The trainer will collaborate with other Community Inclusion trainers to create and publish a monthly schedule of inclusion activities and will organize and carry out those scheduled activities. This role requires communicating and coordinating inclusion activities with clients, families, and necessary staff to ensure successful participation. The trainer will monitor client behavior and implement and complete any required program documentation or program work. The Community Inclusion Trainer may attend staff meetings as needed and participate in agency-sponsored activities. Responsibilities also include transporting clients to and from community inclusion activities. The trainer will perform other related duties as delegated by the supervisor or manager.

*Minimum Requirements: A minimum of a 2-year degree or 2 years' experience in the developmental disabilities field.

 

Client Medical Services Manager, RN or LPN

Pay Range: $35.00 - $40.00

Job description:

The CMS Manager will be responsible for the medical administration of prescriptions and over-the-counter drugs. This RN or LPN will serve as a liaison between the medical services staff, group homes, supported living, and ADT staff and the physicians and pharmacy, as needed. In an effort to coordinate needs and services, the nurse will supervise the medical services staff to oversee the implementation of all medical and therapeutic interventions. Will coordinate with other medical staff for on-call duties as required.

Other responsibilities:

-Training staff in medication administration through an APD-approved course, as well as providing CPR and First Aid training.
-Oversees the medical administration of prescription and over-the-counter medications and supervises medical services staff in the implementation of medical and therapeutic programs.
-Transporting clients to/from medical appointments when needed, reporting incident reports to the Agency for Persons with Disabilities and following up as required, and monitoring Group Home Medication Administration Records while reporting any medication errors to APD.
-Documenting appointments and follow-up information in Therap or other agency-required systems and maintaining up-to-date medical face sheets for administrative staff, group homes, and the Supported Living Manager as needed.

*Minimum Requirements: Four (4) year college degree in nursing, or two (2) year nursing degree with the equivalent of two years’ experience in the field. Active RN or LPN license with the state of Florida.

Preferred Experience: Proficient in computer skills or willing to become proficient with outside programs such as iConnect and Therap.
